I tend to be fairly patient with the unobservant, as I myself have made some pretty spectacular spot-check fails in my time. So long as the person recognizes that they've screwed up I can always forgive. Now, a person who fucks up and tries to avoid taking responsibility out of embarrassment or some bizarre from of pride, that irks me.
I tend to be more cautious with the use of the term berserk button as I actually have them to full effect - as in the mindless, thoughtless rage variety. But my personal pet peeve is someone telling me something I already know. It's always annoying even if they don't know I know, but it really pisses me off if they either know I know, or it's obvious. It feels like an insult to my intelligence, which I've always taken some pride in. Particularly because I look rather stupid to most people. Someone telling me to do something I'm doing or planning to do has the same effect, really.
