If we didnt have the Cakes' babies to prove otherwise I'd say they wait until the kid starts showing traits before they name them.

Maybe they use magic to divine the basic nature of the foal and name them based on that?

I believe that all ponies have a name given to them when they are born and a name given to them when they aquire their marks. For example, we never see a character called by their name in the show during the flashbacks in Cutie Mark Chronicles. Also, this could be used as a You Called Me X, It Must Be Serious scenario, since only a pony's truest friends can know his or her birth name. Using a pony you don't know or like's birth name is a major insult.

Also, has a pony ever stated that they hated another pony in the show? Because that could play a major part in my headcanon.

[up]Due to Twist's scenario, disregard everything I just said. Then again, Twist could've still been picking out her name, and the nicknames used for the other fillies could be placeholders, due to the fact that childhood enemies could misuse the birth name like that as well.

This person is a really fun artist.

Plus there's a lot of ponies whose names could really be seen as not related to anything to do with their talent. Applejack? Well, all the apple family are named off apple products. Pinkie Pie? unusual colour for the fasmily, makes sense they'd give her a name to do with it. Twilight Sparkle: Born during the evening. Rainbow Dash? Rainbow mane and tail.

Not to mention the side ponies, like Junebug or Cheerilee. Heck, to be honest there isn't even that much in the way of super meaningful names for the ponies. The different races just seem to have decent enough naming conventions that, to us, seem like they're very telling. but keep in mind most of our names are based on real words, just very very old ones. So at one time human names may have been akin to pony names.

@Apple: While surnames are more obvious, I actually meant given names. think about it: All names have a meaning of some sort, right? Why would we say they have a meaning if they were not once the word for what they mean? The progression of language has likely simply turned what were once real words into memories, but since we use them as names they persist even if their original use was lost.

Of course even then there are some pretty obvious ones. Names like Robin, or Asher, or Brook.

I'd like to think that a pony could change their name quite easily if they wanted to. Heck, Pinkie never uses her full name. Stuff like Diamond Taira though is pure Self-Fulfilling Prophecy. She was named like a little princess, treated like a little princess, and her talent appears to be bossing ponies around, like a little princess.
