@Story: It doesn't actually. There are certain physiological effects from castration, depending on when it was done(Ie. before or after puberty) - before puberty leads to distended limbs and an enlarged chest cavity, as well as more generally childish or feminine features, after puberty mostly just leads to the relocation of body fat the chest and hips as opposed to the stomach. I've also heard that having it done after puberty leads to the voice heightening, but I'm not certain about that. Either way, Castrato is the general term for voices of castrati who were made as such before puberty, anyways.

It should be noted that it can still be effectively impossible to tell a castrati at first glance, regardless of when they were castrated. The lack of testosterone does not necessarily mean they become mutants or look like women, and a castrati can in fact look quite masculine. The guards of a sultans concubines were largely castrati from birth, and they were, of course, quite large.

@ginger: there is a difference between a feminine man and having your rocks lopped off. For the first point, as I'd mentioned being castrated does not necessarily make one look feminine. Secondly, of course it's going to invoke some sort of castration anxiety, I'm a guy after all. That's just instinctive. It's one thing if she had been gelded at birth and thus didn't know what she was missing(though she's demonstrated a sex drive which makes that unlikely, if not impossible), but the thought of it still makes me a bit uncomfortable. It seems unfair to take away their chance like that.
