Pinkie Pie's conversation with the cow in A Friend in Deed implies that the cows milk themselves. Now, as for how they do it, I have no idea. They probably have a tool for it.

Aren't there milking machines? Considering the tech level in Equestria, there's no reason they couldn't have those. I mean, you still have to put them on, but it's way less awkward.
