If you're talking the Big Mac-with-Caramel's-colors & cutie mark, he's always been there. I thought he might Big Mac's original design, but Lauren Faust's pitch sketch shows his cutie mark didn't change.

Actually, using the cheerilee example, I don't think cutie marks mean a damn thing. It's just a picture, one you can interpret it in any way that you want.

Still, at the very least, the marks are atleast vaguely related to what they do, right?

Sure not all of Rarity's dresses have gems on them, but she often still accesorises outfits with them. And she clearly has a... Thing for large gems, or delusions of them.
