Derpy shows up in three shots of town square: twice with Doctor Whooves (in the episode's first shot and in a shot during the song; both shots use the same background pony positioning) and once with Tornado Bolt (in another shot during the song).

As for Cheerilee and Big Macintosh, these

printables seem to indicate that there really is something between them. (Also, since the first one refers to Big Macintosh as BIG MAC®, it looks like any potential issues with McDonald's have been worked out.)

Yeah, the ending is a bit ambiguous. It could be entirely possible that Big Mac and Cheerilee are just messing with the CMC, but it's also implied that they're going out anyway and that the CMC trolling was just the mushy nicknames.
