Considering it's in the middle of the forest, perhaps it was created from a regular pool, which might have been tainted by the local magic, by a unicorn that got lost in the forest and tried not to go mad from isolation any way he could?

Huh, I don't know what the story behind the pool is. Given the kind of weird stuff that happens in Equestria, it's possible that it could be a natural phenomenon. I mean, you have plants that play jokes and magical artifacts that embody emotions and ideals, so anything's possible I guess.

My personal canon is that the Everfree Forest was originally an "ordinary" forest. In which a castle most often used by Luna was located. I like to think that its original name was the Forest of Midnight or something of the sort.
In any case, when Luna... turned, I imagine that the first fight between Celestia and Nightmare Moon began in that castle. The duel caused such an enormous and catastrophic clash of magical energies that it changed the forest forever. Freeing it, permanently, from the mandated cycle of nature set up by the equine races.
Hence, the Everfree Forest.

I like to imagine the Castle Ruins in the Everfree are the remains of Old Canterlot, the previous castle/capital of Equestria.
