My take on CMFIS isn't that it's a disease or something that causes the breakdowns, but rather than somebody simply recognized a patterns that sometimes occurs and assigned a name to the phenomenon. And it doesn't require the cutie mark to necessarily indicate purpose, either. Even if we assume that it simply indicates an affinity for a particular skill (which I think it does), it's still understandable that seeing the value of that skill called into question could lead to an identity crisis. Even if Twilight's CM simply represents "is good at and enjoys magic", it's still entirely possible that a horrible failure of her magical ability could still cause her emotional pain.

TL;DR CMFIS doesn't cause breakdowns, it's just a name given to the circumstance in which self-doubt over a cutie mark causes a breakdown.

I remember seeing a fic on EQD about Twilight and Luna going in to Celestia's dreams to try and wake her up from a coma or something.
