The game mkes it canon that a powerful enough Magical Girl can wish to turn a witch back. And the Elements of Harmony are more powerful than Discord. I don't think even second-to-last timeline Madoka was as powerful as Discord.

I doubt that even The Elements of Harmony can defy Death itself and recreate the shattered and twisted fragments of a broken soul. Even Luna's soul was intact, if corrupted, in her Nightmare Moon state. The Incubator-enabled Wishes of Magical Girls can defy physics and the rules of reality because they appear to run on the same Reality Imposition effect the Espers in To Aru Kagaku no Railgun use to impress their reality-bending powers on the world and are subject to similar limitations and requirements.
@Kyler: In either one of the paths in the Madoka PSP game or one of the alternate timelines in the sidestory manga, Madoka canonically used her Wish to undo Sayaka's Witchification and return her to "normal".
